The One Paw at a Time Adoption Application is a legal document and must be submitted in an unalterable

format. All applications must be submitted electronically in order to be processed.

Please submit one of the following ways:

• Fill out customizable PDF (Located Below), save to hard drive, and email back to us as an attachment (onepawatatimerescue@gmail.com)

• Print out and scan the document and email back to us as an attachment

• Print out and take a picture of each page so that the document looks as though it was scanned.

Pictures must be taken straight on. NOTE: Pictures taken at an angle or with cut off portions of the

document will not be accepted and are considered incomplete. You will be asked to resubmit the application.

• Download a free scanner app on your phone and take pictures of the document

(Please note that we do not have a fax machine)

We accept applications on a first come first serve basis so time is of the essence if you find a dog on our site that you would like to adopt. Please double check that you have completed the full application, the manner in which you are submitting the application is in the proper format, and that all areas are filled out fully before submitting. Specifically please ensure that the vetting information, animals in your home, references and landlord information are fully and properly filled out.

One Paw at a Time Rescue does not accept incomplete applications and the delay of going back and forth could cost you the adoption opportunity for the dog you’re applying for. If you have or have had animals within the last 5 years please reach out to your vet and let them know we will be calling and that it is okay to speak with us.
